As Co-Chairs of the Eastern York Region North Durham Ontario Health Team (EYRND OHT), we are pleased to present the Annual Report for the fiscal year 2023-2024. This report encapsulates our team's commitment, achievements, and ongoing dedication to improving health care delivery and outcomes for the residents of Markham, Stouffville, Uxbridge, and beyond.

Meet Imogen

Imogen, a resident of Markham found herself in a tough situation when the ligaments in her knee where torn after a fall. With her orthopedic physician retiring, she wasn’t sure what her options were. Thankfully, the Central Region Virtual Urgent Care Clinic offers accessible health care and was given a requisition to get an X-ray at Markham Stouffville Hospital.

Seniors home support

This program delivers team-based primary care to homebound seniors living at home in the communities of Markham and Stouffville. It is one of Ontario’s few home-based primary care programs with community paramedicine integration. In 2023-2024, the team fulfilled 100 per cent of patient and family requests for urgent home visits in the same or next day.
