Primary Care Network

Who we are

Our Primary Care Network represents over 200 family physicians with practices or patients in the EYRND OHT catchment area of Markham, Whitchurch-Stouffville, and Thornhill communities. Our membership includes a diverse variety of practice models, sizes and regions.

Our vision is for integrated care for everyone in the Eastern York Region and North Durham communities.

Our mission is to advocate for improvements to the health care system that benefit the patients we serve and to enhance primary care's central role in the well-being of our community.

Our values are inclusivity; frank, open and honest discussion; and factual, evidence-based decision-making.

About SCOPE (Seamless Care Optimizing Patient Experience)

SCOPE supports primary care providers by improving equity and access to care for their patients in the EYRND OHT communities. Oak Valley Health and community partners are proud supporters of the program.

As a single point of contact, a nurse navigator connects and navigates primary care providers to a multitude of services for their patients, efficiently and timely.

Services include: GIM, Ontario Health at Home, Nurse Navigator, DI, COVID at Home, Obstetrics and Gyne, Wound Care, Mental Health, Oncology, Gastroenterology, Plastics and Home First.

Primary care providers can join the SCOPE program with no obligation by visiting scopehub.ca.
