Integrated Transitional Care Program

The Integrated Transitional Care Program is designed for patients that are medically stable and can be safely transitioned back from Oak Valley Health (formerly Markham Stouffville Hospital) into the community and/or an appropriate congregate setting. The Integrated Transitional Program will ensure a smooth and seamless experience for eligible patients  by providing wrap-around services through partnerships with home care and local community support organizations. This program is designed to empower caregivers and support patients in the community.
