Who we are
Our Primary Care Network represents over 200 family physicians with practices or patients in the EYRND OHT catchment area of Markham, Whitchurch-Stouffville, and Thornhill communities. Our membership includes a diverse variety of practice models, sizes and regions.
Our vision is for integrated care for everyone in the Eastern York Region and North Durham communities.
Our mission is to advocate for improvements to the health care system that benefit the patients we serve and to enhance primary care's central role in the well-being of our community.
Our values are inclusivity; frank, open and honest discussion; and factual, evidence-based decision-making.
The OHT is an innovative approach to health care that relies on an integrated model and uses best practices from clinical guidance and research. The Primary Care Network’s goal within the OHT is to ensure all primary care providers within our community have access to the tools and resources necessary to provide seamless care. Now is a critical time in early OHT development where we need your support and your voice to build the vision for better health care in our community.

Why me?
You, as a primary care physician, play a pivotal role in shaping health care. We need the support and engagement of physicians such as yourself to create a fully integrated health care system. By collaborating with hospitals, patients, caregivers, and family doctors such as yourselves, we hope to achieve a coordinated model of seamless care.
Is there a cost?
No! We want to ensure you there are no hidden fees or penalties to joining the Primary Care Network. It is a voluntary association and you can choose to leave the group at your will.

About SCOPE (Seamless Care Optimizing Patient Experience)
SCOPE supports primary care providers by improving equity and access to care for their patients in the EYRND OHT communities. Oak Valley Health and community partners are proud supporters of the program.
As a single point of contact, a nurse navigator connects and navigates primary care providers to a multitude of services for their patients, efficiently and timely.
Services include radiology, GIM, mental health, fracture clinic, obstetrics, clinical assessment center, COVID 19 therapeutics, home and community care and other services in the community.
Primary care providers can join the SCOPE program with no obligation.

Primary Care Advisory Council (PCAC)
The Primary Care Advisory Council (PCAC) is responsible for developing recommendations for the integration of primary care into the EYRND OHT. The function of the PCAC includes:
Sharing knowledge, experiences, and best practices pertaining to primary care
Providing recommendations to the EYRND OHT with regards to standardization, coordination, access, and quality of care
Supporting the implementation of the EYRND OHT initiatives
Advising the EYRND OHT Core Leadership Council on primary care strategic direction
Engaging local primary care colleagues to gather feedback and advise PCAC and communicate key EYRND OHT messages in information back to the primary care community
Recently, the Primary Care Advisory Council surveyed primary care providers within the catchment area. The purpose of the survey was to conduct a needs assessment of the gaps/barriers that primary care providers experience when managing patients with mental health, addictions-related, or dementia issues.
