Patient, family, caregiver, and community advisory committee

About the Patient, Family, Caregiver, and Community Advisory Commitee:

As a Patient, Family, Caregiver, and Community (PFCC) partner with the EYRND OHT, you will be included in a group of partners from various backgrounds that will be involved in various committees, projects, and/or initiatives within the OHT. If selected, you will be asked to attend meetings as required and/or contribute your personal experiences, knowledge, and feedback for particular projects.

We are looking for individuals who are:

• Available to actively attend and participate meetings virtually (typically between the hours of 9 am to 5 pm). However, a majority of the work will require your feedback to be completed via email and outside of regular work hours.

• Can work with a diverse group of individuals including youth, caregivers and community partners.

• Have lived experience using healthcare in the various settings: family doctor visits, emergency department visits, mental health and addictions programs/services, long-term care and retirement homes, community programs, and much more.
